New Patients

Welcome to Riverside Pediatrics!  We would be honored to be your child’s pediatricians and are delighted that you have chosen to join the Riverside family.  If you have any questions or require any assistance, please feel free to contact the Office Manager at the location that you visit.  Also, you may contact our Patient Care Coordinators at any time to assist with your needs. 

This section includes the New Patient Registration Package, which includes the forms that you will need to complete prior to your first visit.  It also includes other forms, such as our Record Release Form.  If it is your first visit to Riverside Pediatrics, please bring a completed copy of the New Patient Registration Package with you to your first visit to minimize your wait time.
